A Bridge to Recover for Older Adults Returning Home from Sequoia Hospital
January, 2011

Menlo Park, CA (January, 2011) - Peninsula Volunteers received a $25,000 second year grant from CHW/Sequoia Hospital for the local collaboration known at the Sequoia Hospital Homecoming Program (SHHP). Peninsula Volunteers Meals on Wheels is providing home delivered meals to newly discharged patients from Sequoia Hospital who are 50 years old and over and have little or no support at home. Other community organizations in the collaboration include Peninsula Family Service, which is doing case management and referrals, Samaritan House, for assisting with medication management and education, , for weekly grocery delivery, and Rebuilding Together Peninsula for free home repairs and modifications.
